Setup PIHM for North Temperate Lakes

Plans for how to integrate lakes with PIHM, progressively more complex:

  1. Constant lake level
  2. Time variable boundary conditions (weakly coupled lake model)
  3. Sequential coupling (2 way with lake) (help from Gopal?)
